29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical agents: The employer is failing to ensure that the eyes and face of employees are protected from flying particles while working on a residential roof. This most recently occurred on October 29, 2018 on the jobsite located at 1255 Royal Dr., Papillion, NE 68046. Two employees were observed using hand-held pneumatic nail guns to install roofing material without any form of eye or face protection.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: The employer is failing to ensure that all employees exposed to fall hazards while working on a commercial roof are utilizing fall protection. This most recently occurred on October 29, 2018 on the jobsite located at 1255 Royal Dr., Papillion, NE 68046. The company owner and six employees were re-roofing a commercial building and not utilizing any form of fall protection. The building has roofs that range from four in twelve to sixteen in twelve and the ground to eve height is approximately eight and a half feet.
