29 CFR 1910.134(g)(1)(i)(B): Respirators with tight-fitting facepieces were worn by employees with a condition that interfered with the face-to-facepiece seal or valve function: (a) Burnham, LLC - The employees who wore North 7190N99, or Moldex 6200 half-mask respirators for protection against crystalline silica quartz had facial hair which interfered with the face-to-facepiece seal, on or about October 30, 2018. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested whenever a different respirator facepiece (size, style, model or make) was used or annually: (a) Burnham, LLC - During the inspection on October 30, 2018, the refractory used tight-fitting respirators for protection against respirable crystalline silica (silicosis hazard); however, they were not fit tested in within the previous twelve months and used different respirators than they were fit tested for. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
