29 CFR 1910.119(h)(3)(iv): The contract employer shall assure that each contract employee follows the safety rules of the facility including the safe work practices required by paragraph (f)(4) of this section. a) Delaware City Refining Company, 4550 Wrangle Hill Road, Delaware City, Delaware, Unit 22 Coker (Department), D2A Fluid Coker Reactor, Cuff Feed Nozzle 9th Deck South Side- On or about October 30, 2018, the employer failed to ensure that employees were following the safe work practice for removing a cuff feed nozzle from Coker No. 22 for maintenance.
