29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: 997 Western Ave, Westfield, MA: On October 31, 2018 and at times prior, employees who changed blades on the band resaw machines did not use the employer-developed energy control procedure that required locking the electrical energy source. Employees changing the blades only activated emergency stop buttons that did not physically isolate or lock the electrical energy.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: 997 Western Ave, Westfield, MA: As of October 31, 2018 and for many years prior, the employer had not performed a periodic inspection that ensured each procedure was inspected for inaccuracies during an actual energy control task. The employer also did not ensure that the inspector reviewed with each authorized employee their responsibilities under each energy control procedure.
