29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, and rotating parts: A) EMI Enterprise, Inc., doing business as Envelope Mart USA, located at 2639 Tracy Road, Northwood, OH 43619: On or about October 22, 2018, the employer did not ensure that one or more methods of adequate machine guarding were provided to protect employees from the hazards such as, but not limited to, pinch points between the housing of the folding blade and the table of the Stoesser plate bending machine located in the Prepress Room. Employees were exposed to caught-in-between and amputation hazards.
