29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: JOB SITE: 1203 S. Babb Lane, Salem, MO 65560, on or about 10/31/2018, workers were given an extension cord to power temporary lighting in the service bay, which was missing a ground prong on the power cord. As a result, the path to ground from the equipment was not permanent, continuous, and effective which caused the employees to be exposed to an electrical hazard.
