29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:   JOB SITE: 1203 S. Babb Lane, Salem, MO 65560, on or about 10/31/2018, workers were given an extension cord to power temporary lighting in the service bay, which was missing a ground prong on the power cord.  As a result, the path to ground from the equipment was not permanent, continuous, and effective which caused the employees to be exposed to an electrical hazard.

29 CFR 1910.305(g)(2)(ii): Flexible cords were not used in continuous lengths without splice or tap:  JOB SITE: 1203 S. Babb Lane, Salem, MO 65560, on or about 10/31/2018, in the service bay an electrical cord was spliced to power two (2) fluorescent lighting units.  As a result, the flexible cord was not used in continuous lengths.  The workers were exposed to an electrical hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  JOB SITE: 1203 S. Babb Lane, Salem, MO 65560, on or about 10/31/2018, the service shop had not ensured a 220v electrical outlet had proper strain relief on the wall of the service bay. The workers were exposed to an electrical hazard.

29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a Hyster Forklift:   JOB SITE: 1203 S. Babb Lane, Salem, MO 65560, on or about 10/31/2018, the employer failed to ensure each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training.  The workers were operating the Hyster Forklift without the required training.
