29 CFR 1910.23(e)(1): A standard railing shall consist of top rail, intermediate rail, and posts, and shall have a vertical height of 42 inches nominal from upper surface of top rail to floor, platform, runway, or ramp level.      The stairway to the supervisors office did not have a mid rail installed exposing employees to fall hazards up to 15 feet.

29 CFR 1910.215(a)(4): Grinding machinery was not used with work rest(s) to support offhand grinding work.        Bench grinders used by employees were not equipped with work rests exposing employees to caught-in and struck-by hazards.  The employer did not ensure all equipment used to aid in shaping and/or grinding material was inspected, such as but not limited to:    a)  In the press break area, the floor grinding & sanding machine was not equipped with work rest,  b)  In the West Well area, the DeWalt table grinder was not equipped with work rest adjusted less than 1/8 inch to wheel,  c)  In the press brake section, the DeWalt table grinder was not equipped with work rest.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch.     The employer did not ensure adjustable tongue guards were installed and used prior to operating grinders, exposing employees to struck-by flying objects.   The employer did not ensure all equipment used to aid in shaping and/or grinding material was inspected, such as but not limited to:  a)  In the press break area, the floor grinding & sanding machine was not equipped with adjustable tongue guard, b)  In the West Well area, the DeWalt table grinder adjustable tongue guard not adjusted, c)  In the press brake section, the DeWalt table grinder adjustable tongue guard not adjusted.
