Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) Jobsite, 2021 West Chestnut Street, Washington, PA: On or about October 30, 2018, three employees were installing felt paper on a commercial roof at heights of 14-30 feet above the ground and were not utilizing fall protection. The roof pitch was 4 in 12.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer: a) Jobsite, 2021 West Chestnut Street, Washington, PA: On or about October, 30, 2018, three employees were installing felt paper on a commercial roof at heights of 14-30 feet above the ground without utilizing fall protection. The owner said he trained them years ago and talks to them about safety, but did not have any written documentation of fall protection training.
