29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: On or about October 30, 2018 an employee was using a damaged lanyard which was not inspected and taken out service prior to use. The employee was exposed to hazards associated with falls and insufficient fall protection due to wearing a damaged lanyard as fall protection while working on a roof. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about October 30, 2018 at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
