29 CFR 1926.501(b)(11): The employer did not ensure that  each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    (a)Centro Home Improvements at 7505 West Boysenberry Street, Sioux Falls, SD 57106: On or about and at times prior to October 31, 2018, the exposing employer did not ensure employees performing roofing activities at heights greater than six feet above ground level were protected from fall hazards in that, employees performing roofing work did not utilize an appropriate fall protection system. This condition exposed employees to fall hazards greater than six feet above ground level.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written certification record:  (a)Centro Home Improvements at 7505 West Boysenberry Street, Sioux Falls, SD 57106: On or about and at times prior to October 31, 2018, the exposing employer did not verify that employees were provided training on the recognition of fall hazards and appropriate procedures to be followed to minimize exposure to these hazards by creating a written certificate record.
