29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end.    Location - 347 Pleasant St. Dracut    On or about October 31, 2018, employees were exposed to from falls approximately 18 Ft to a lower level off of a cantilevered scaffold platform. The cantilevered platform extended greater than 18 in. beyond the scaffolds end support.

Location - 347 Pleasant St. Dracut    On or about October 31, 2018, employees were exposed to falls greater than 10 feet to a lower level on a ladder jack scaffold.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.    Location - 347 Pleasant St. Dracut    On or about October 31, 2018, ladders used to gain access to a upper level and supporting ladder jack scaffolding were not free from defects.    Instance 1: An aluminum extension ladder supporting scaffolding had two bent rungs.  Instance 2: An aluminum extension ladder used to gain access to the ladder jack scaffold had two bent rungs.
