29 CFR 1926.451(g)(4)(i): The employer did not ensure that guardrail systems are installed along all open sides and ends of platforms. On or about October 29, 2018, employees working on a single story commercial building were exposed to falls of approximately 12 feet to the ground below while working from an elevated work platform. A complete guardrail system or other form of fall protection was not in place.

29 CFR 1926.501(b)(1): The employer did not ensure that each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. This violation occurred on October 29, 2018, where at least three (3) employees working on the roof without fall protection and exposed to falls of more than 44 feet to the ground below.
