29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    a) 5369 Route 9 North, Howell NJ:  Company worksite employees were exposed to fall hazards in excess of 25 feet to the ground surface when working/walking from a supported scaffold while utilizing life lines improperly installed and not used as per the manufacturer's specifications by the company employees along sections of the scaffold that were not supplied with mid and top rails or missing combinations of each. Observed on or about 10/31/18.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
