29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, in-going nip points, rotating parts, flying chips and sparks:   Facility Maintenance Shops: In the following locations, the employer did not provide point of operation and rotating parts guards. Employees were exposed to severe lacerations and amputations.   1) Wood Shop: A Delta Rockwell 10 inch table saw and a Delta Milwaukee Radial saw were not provided with guards to protect the operator from hazards created by point of operation.  2) Auto Maintenance Garage: A Sears / Craftsman 20" drill press was not provided with a guard to protect employees from flying chips and sparks.  3) Machine Shop: A Dayton 15" drill press was not provided with a guard to protect employees from flying chips and sparks.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one-fourth inch:  In the following locations, two 10 inch bench grinders had the distance between the abrasive wheel periphery and the adjustable tongue guard not adjusted to the maximum permitted opening of one fourth inch.  1) Facility Maintenance Machine Shop: A Thor bench grinder tongue guard was measured one inch and a quarter of an inch from the abrasive wheel.   2) Automotive Garage: A Dayton bench grinder tongue guard was measured two inches and a quarter of an inch from the abrasive wheel.
