29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:      (a) Randy Carafagno Productions. Employees,  in the process of manufacturing the puppets, are provided and required to wear half face tight fitting respirators. The employer did not develop or implement a respiratory protection program. The condition was noted at or around October 31, 2018.         NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 AS PER 29 CFR 1903.19.

The condition was noted at or around October 31, 2018. The employer did not have the  safety data sheets for the chemicals used at the site. The condition was noted at or around October 31, 2018.
