29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:   A)     J.H. Routh Packing Company, located at 4413 West Bogart Road, Sandusky, OH 44870: On or about October 22, 2018, the employer did not ensure that periodic inspections of the energy control procedures for the cutting wheels at the Casing Line were conducted at least annually. Employees did not follow all the steps required for the control of the hazardous energy sources prior to conducting activities such as, but not limited to, unjamming the casing from the cutting wheels. Employees were exposed to hazards that could result in personal injuries while all hazardous energy sources were not controlled prior to performing these tasks.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:     A)     J.H. Routh Packing Company, located at 4413 West Bogart Road, Sandusky, OH 44870: On or about October 22, 2018, the employer did not adequately train employees to lockout/tagout equipment such as, but not limited to, cutting wheels at the Casing Line. Employees did not control all hazardous energy sources such as, but not limited to, electrical and kinetic before conducting activities such as, but not limited to, unjamming the casing from the cutting wheels. Employees were exposed to hazards that could result in personal injuries while all hazardous energy sources were not controlled prior to conducting work.

Routh Packing Company, located at 4413 West Bogart Road, Sandusky, OH 44870: On or about October 22, 2018, the employer did not ensure that all the steps outlined in 29 CFR 1910.147(d), required for the control of the hazardous energy sources, were performed. Employees did not deenergize the cutting wheels on the Casing Line prior to unjamming the casing from the cutting wheels. Employees were exposed to the hazards that could result in amputations while all hazardous energy sources were not controlled prior to performing these tasks.

Routh Packing Company, located at 4413 West Bogart Road, Sandusky, OH 44870: On or about October 25, 2018, the employer did not ensure that the point of operation of the Comb I (rollers station # 2), located at the Casing Line, was adequately guarded. Employees were exposed to hazards that could result in amputations in that employees were able to reach through an opening of approximately 2.75 inches into the rollers (point of operation).
