29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    Semi-metallic, hot press #1: on or about November 1, 2018, employees assigned to handle hot brake shoes with hoisted grip tongues and handheld scrapers from the furnace, in/out of the die box, and to/from plug press station were exposed to contact with hot surfaces and flying sparks likely to result in burn injuries because heat-resistant work aprons and/or other personal protective equipment (PPE) were not provided to protect body parts from inadvertent direct physical contact.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(3)(iii): Special handtools for placing and removing material shall be such as to permit easy handling of material without the operator placing a hand in the danger zone. Such tools shall not be in lieu of other guarding required by this section, but can only be used to supplement protection provided.    Semi-metallic, hot press #1: on or about October 17, 2018, operators assigned to remove individual hot brake shoes from the die box's insert were not provided with adequately designed and constructed handtools to permit safe removal. Used scraper was not long enough to ensure safe distance between the operator's hand and up/down moving insert when loosening the brake shoe for removal. The scraper could fall into the die box openings created during the up/down movements and pull the operator's gloved fingers down and towards a pinch point because of the scraper's curved end as evidenced by the amputation injury accident.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.    Anchor Brake Shoe Company, LLC in West Chicago, Illinois: on or about October 17, 2018, the employer failed to report a work-related fingertip amputation to OSHA, U.S. Department of Labor within 24 hours after the accident occurred in the workplace.    Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
