The written program must be made available prior to and during entry operations for inspection by employees and their authorized representatives. a) On or about October 29, 2018 at 2001 Riverside Drive, Gainesville, Georgia, an employee was directed to enter into a utilities manhole to determine if the conduit in the manhole serviced the Curtis Building where repairs to a data conduit were being made. The employer did not document or implement a written permit space program as required.

The employer failed to provide a safe means of access/egress in a trench that was approximately 9 feet deep and 3 feet wide.
