Located at 3610 West Pike, Zanesville, OH: On or about October 25, 2018, the employer did not ensure that an employee was protected from falls while cleaning on top of the horizontal (textile) baler with unprotected sides, thereby exposing the employee to a fall hazard of approximately eight feet to the concrete floor.  b.  Located at 3610 West Pike, Zanesville, OH: On or about October 25, 2018, the employer did not ensure that an employee was protected from falls while cleaning on top of the horizontal (cardboard) baler with unprotected sides, thereby exposing the employee to a fall hazard of approximately seven feet to the concrete floor.

Located at 3610 West Pike, Zanesville, OH: On or about October 25, 2018, the employer did not develop nor document procedures for the control of hazardous energy when employees were engaged in the cleaning activities to the balers.  Equipment covered by this part included the horizontal (Maren, Model: 72/60-301, Horizontal Closed Door Baler) and vertical (CRAM-A-LOT, Model VB-72) balers for servicing activities.  Employees were exposed to potential struck by and caught-in hazards, during the cleaning of the balers.

The training shall include A through C.    a.  Located at 3610 West Pike, Zanesville, OH: On or about October 25, 2018, the employer did not provide training to ensure that employees were trained on the skills required for the safe application, usage, and removal of energy controls for the baler machines.  The baler machines at the facility included the horizontal (Maren, Model: 72/60-301, Horizontal Closed Door Baler) and vertical (CRAM-A-LOT, Model VB-72) balers.  Employees were not trained as authorized employees and thereby, were exposed to potential struck by and caught-in hazards, while servicing the baler machines.

29 CFR 1910.147(f)(3)(i): When servicing and/or maintenance is performed by a crew, craft, department or other group, they shall utilize a procedure which affords the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device.     a.  Located at 3610 West Pike, Zanesville, OH: On or about October 25, 2018, the employer did not have a procedure in place to afford employees protection from the control of hazardous energy (lock-out, tag-out) while cleaning the balers at the facility.  The employees cleaned the balers, while not having exclusive control for their safety executing this task, thereby exposing the employees to a caught in and struck by hazard.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed.   a.  Located at 3610 West Pike, Zanesville, OH: On or about November 01, 2018, the employer did not ensure that exposed energized wires were protected from abrasion and the opening effectively closed into the electrical machine motor for the textile baler.  The electric conduit was pulled out of the opening leading into the motor housing.
