29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed:  On or about 10/31/18, the employer failed to ensure that a periodic inspection was completed, at least annually, of the lockout/tagout procedures. Employees were tasked with conducting maintenance and repair activities on machinery, including but not limited to the Cincinnati Milacron VS - 85 - 4.44 S85A0400003 plastic injection molding machine.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  On or about 10/31/18, the employer failed to ensure that employees operating the forklifts, were provided with operator training, in accordance with 29 CFR 1910.178(l).
