29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed: On or about 10/31/18, the employer failed to ensure that a periodic inspection was completed, at least annually, of the lockout/tagout procedures. Employees were tasked with conducting maintenance and repair activities on machinery, including but not limited to the Cincinnati Milacron VS - 85 - 4.44 S85A0400003 plastic injection molding machine.
