29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about 10/29/2018, at 2161 Frangipani Circle, in Naples, Florida, employees were standing on a wall without fall protection exposing them to a fall hazard of approximately 10-feet.

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top.  On or about 10/29/2018, at 2161 Frangipani Circle, in Naples, Florida, employees used a ladder that did not extend at least 3-feet above the landing exposing them to a fall hazard of up to 10-feet.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  On or about 10/29/2018, at 2161 Frangipani Circle, in Naples, Florida, a defective extension ladder was used for access and not tagged and removed from service exposing employees to a fall hazard.
