29 CFR 1926.451(g)(1)(iv): Each employee on a self-contained adjustable scaffold was not protected by a guardrail system. a) On or about October 26, 2018 at the address of 33000 Pin Oak Parkway in Avon Lake, OH: The employer did not ensure an employee was protected from falls while working from a supported scaffold platform. The employee was exposed to a 12ft fall to the level below while performing drywall finishing work, near the unguarded end of the scaffold platform. An employee suffered a severe leg injury when he fell from the scaffold platform.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. a) On or about October 26, 2018 at the address of 33000 Pin Oak Parkway in Avon Lake, OH: The employer did not ensure an employee working from a scaffold platform, was trained in the recognition of hazards associated with this type of scaffold. Fall hazards went unrecognized while an employee performed drywall finishing operations from the scaffold platform.
