29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition:  a. Banbury Mixer No. 3, Half Floor - The employees were exposed to a slip, trip and fire hazard.  On or about October 31, 2018, Housekeeping around the Banbury mixer at the half floor needed improving. A mixture of grease, carbon black and other items were on the floor around the mixer.  The Banbury mixer warning signs, dangers signs and labels, if any, were obscured by a thick coat of grease and carbon black.  The grease on the outside of Banbury mixer could constitute a fire hazard if electrical parts overheat or moving parts overheat due to friction.

On or about October 31, 2018 the Banbury mixer operator attempted to clean rubber from the discharge door without locking out the mixer and inserting/tightening the safety bolts into the top of the discharge door.
