29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. (a) Harris McBurney on or about October 20, 2018, at the address of 509 Moore Avenue, Bellevue, Ohio: The employer failed to report an employee hospitalization following a worksite incident (Struck-by telecommunications pole). The employer or employer representative failed to contact the Toledo Area Office or the 24 hour OSHA number to notify the agency of an employees hospitalization within 24 hours of admittance. The employer later contacted the OSHA Office on October 29, 2018.
