On or about October 30, 2018, at the above addressed jobsite, an employee was not trained in the recognition of jobsite hazards, which include but not limited to, the hazards related to residential construction.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.    a.  On or about October 30, 2018, at the above addressed jobsite, an employee conducting concrete pouring operations was not provided training on how to minimize fall protection hazards, thereby exposing the employee to hazards associated with falls.

On or about October 30, 2018, at the above addressed jobsite, an employee performing concrete form installation operations, ten (10') feet five (5") inches above the ground, was not protected from falls, thereby exposing the employee to fall hazards.
