29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about October 25, 2019, at SW 2nd Street and SW 5th Avenue, Fort Lauderdale, Florida, 33301, the employer failed to ensure an employee wore a personal fall arrest system or travel restraint system, properly attached to the boom or basket of an aerial lift while applying paint to a building, therefore exposing the worker to a fall hazard. NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: On or about October 25, 2019, at SW 2nd Street and SW 5th Avenue, Fort Lauderdale, Florida, 33301, an employee was operating an aerial lift to apply paint to the exterior walls of a building and was not properly trained on the operation of the equipment, therefore exposing the worker to a fall hazard. NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
