29 CFR 1926.451(g)(1)(ii): Each employee on a single-point or two-point adjustable suspension scaffold was not protected by both a personal fall arrest system and a guardrail system: On or about October 25, 2018, at 626 N.E. 4th Avenue, Fort Lauderdale Florida, 33304, employees were not protected from falling by both a personal fall arrest system and a guardrail system while operating a two-point suspension scaffold to apply stucco, caulking, waterproofing and paint to a newly constructed apartment building; therefore exposing the employees to a fall hazard. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: On or about October 25, 2018, at 626 N.E. 4th Avenue, Fort Lauderdale Florida, 33304, the employer failed to train employees in the safe use and operation of Personal Fall Arrest System (PFAS) while using a two-point suspension scaffold to apply stucco, caulking, waterproofing, and paint to a new building in construction; therefore exposing the employees to a fall hazard. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
