29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Lower Roof On or about November, 2, 2018, five roofers were not provided with personal fall protection, standard guardrails, nets, or a roof monitor and were exposed to a 15 foot fall hazard while walking across a flat roof to gain access to a ladder on a 18 foot by 23 foot flat roof. b) Upper Roof On or about November, 2, 2018, five roofers were not provided with personal fall protection, standard guardrails, nets, or a roof monitor and were exposed to a 25 foot fall hazard while installing roof flashing at the unprotected edge of an 18 foot by 60 foot flat roof. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer: a) Roof - On or about November 2, 2018, the employer could not provide written certification of employee fall protection training to the Secretary's representative upon request. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
