29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  For Citation 2, Item 1, issued on 12/21/2018, with an abatement date of 1/22/2019, the employer did not submit documents demonstrating the complete abatement of the cited violation to OSHA.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    The employer did not ensure fall protection was implemented during residential construction operations when workers were exposed to the hazard of falling approximately ten (10) feet off the roof eve when they were installing roof shingles.  Xtreme Xteriors, LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1316496, citation number 2, item number 1 and was affirmed as a final order on 7/12/2018, with respect to a workplace located at 2/4 Lucinda Ave, Belleville, IL, 62221.  Xtreme Xteriors, LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1215855, citation number 1, item number 1 and was affirmed as a final order on 3/30/2017, with respect to a workplace located at Lot #90, Bradstreet Drive, O'Fallon, IL, 62269.  Xtreme Xteriors, LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1217788, citation number 1, item number 1 and was affirmed as a final order on 5/02/2017, with respect to a workplace located at 2629-2631 Eastview Dr, Belleville, IL 62221.
