29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  NB Siding Construction LLC at 2152 Fountain Cir. Erie, CO 80516: On or before 11/1/18 the creating and exposing employer did not protect employees from injury in that a pump jack scaffold was not inspected prior to use at the beginning of each shift or after an occurrence which could have affected the structural integrity of the scaffold. An employee fell 15 feet after the scaffold decking gave way under him. This condition exposed employees to a fall hazard of 15 feet.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person:  NB Siding Construction LLC at 2152 Fountain Cir. Erie, CO 80516: On or before 11/1/18 the creating and exposing employer did not protect employees from injury in that a pump jack scaffold was not erected, dismantled or altered under the supervision of a competent person qualified in scaffold erection, moving, dismantling or alteration. An employee fell 15 feet after the scaffold decking gave way under him. This condition exposed employees to a fall hazard of 15 feet.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  NB Siding Construction LLC at 2152 Fountain Cir. Erie, CO 80516: On or before 11/1/18 the creating and exposing employer did not protect employees from injury in that training was not provided to employees by a person qualified in the subject matter to recognize hazards associated with the use of pump jack scaffolds. An employee fell from a pump jack scaffold due to the decking giving way, as one side of the pump jack scaffold was lowered too far in relation to the other side. This condition exposed employees to an unsafe scaffold and a fall hazard of 15 feet.

Erie, CO 80516: On or before 11/1/18 the creating and exposing employer did not protect employees from injury in that employees involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold were not effectively trained by a person qualified in the subject matter. An employee fell from a pump jack scaffold due to the decking giving way, as one side of the pump jack scaffold was lowered too far in relation to the other side. This condition exposed employees to an unsafe scaffold and a fall hazard of 15 feet.
