29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: Employees were exposed to burns and smoke inhalation hazards. The exit doors, located on the south end of C and D tunnels of the grain elevator, were locked from the inside and required employees to have special knowledge to unlock them in order to evacuate the building. It was not immediately apparent how to open the door at the south west which had a lock or the door at the south east which was held closed with a pin and chain.
