29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:   At a commercial construction jobsite, on November 2, 2019 and at times prior thereto, employees engaged in masonry activities used a Stihl cutting saw to cut brick and other material, and were not wearing eye and face protection.  The employees were exposed to struck-by and eye injuries.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   At a commercial construction jobsite, on the east side of the building employees performing masonry activities from a tubular welded frame scaffold with open sides were not provided with fall protection, exposing employees to fall hazards of approximately 13 feet to the ground below.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.   At a commercial construction jobsite, employees performing masonry activities from a tubular welded frame scaffold were utilizing the cross-braces as a means of access; no ladder was provided for safe access or egress exposing employees to fall hazards of approximately 13 feet to the ground below.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   At a construction jobsite, on the east side of the building, the employees worked from the roof area and at the unprotected edges of the roof without fall protection exposing employees to fall hazards of approximately 16 feet to the ground below.
