On October 30, 2018, employees were exposed to amputation and crushing hazards when performing cleaning and sanitation activities on the de-leafing rollers and coring machines. An employee reached into an unguarded serpentine chain drive for the de-leafing rollers during sanitation and received a four finger amputation. The employer did not ensure energy control procedures were documented and utilized to properly control the residual energy before cleaning the equipment.
