29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) Shiloh Commons Project located at 223 Shiloh Road, Billings, MT 59102: On November 6, 2018 and times prior, employees were exposed to catapult hazard and subsequent fall while working from a JLG aerial lift, model 660SJ, and serial number 0300183747 fro, a height of approximately 28 feet, without using personal fall protection.
