29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: On or about 11/5/2018, at the front yard of Florida Trailer Manufacturing DBA Rocket Trailer, located at 2890 NW 35th St, Miami, FL 33142, an employee was exposed to approximate 11-foot fall hazard when performing water pressure cleaning while working from a metal scaffold which was not secured to the forks and/or mast of a powered industrial truck.
