29 CFR 1910.147(c)(4)(ii)(B): The energy control procedure did not clearly and specifically specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:     On or about 11/05/2018, Breckenridge Materials Company dba Breckenridge of OFallon, located at 1440 W Terra Lane, OFallon, MO, did not have specific procedures to control hazardous energy for the Kemco System.  This system had electrical, water (pressure), and natural gas energy sources.

The last periodic inspection was dated July 10, 2014.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):     On or about 11/05/2018, Breckenridge Materials Company dba Breckenridge of OFallon, located at 1440 W Terra Lane, OFallon, MO, had an employee operating a forklift without having completed the required training.  The operator had never attended training and evaluation for the JCB powered industrial truck.
