Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. a. On or about October 18, 2018, the employer did not have the correct capacity plate on the Yale forklift prior to using the Uline fork extensions. Yale Lift Truck model GCL050VXNVRE083 serial number A910V20149K was equipped with the 72" fork extensions and used to unload trucks at the facility at 1301 Capital Dr. Fond Du Lac, WI 54937.
