Located at 8683 Conestoga Valley in Blacklick, Ohio: On or about November 7, 2018, employees were performing roofing work on a new build residential house. The employer did not ensure that fall hazards were identified and corrected onsite to maintain an effective safety and health program.

Located at 8683 Conestoga Valley in Blacklick, Ohio: On or about November 7, 2018, employees were performing roofing work, such as, but not limited to, caulking, installing drip edge, and installing felt. The employer did not ensure that the personal fall arrest systems were being utilized, thereby exposing employees to fall hazards up to approximately twenty foot.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached. a. Located at 8683 Conestoga Valley in Blacklick, Ohio: On or about November 7, 2018, an employee was installing drip edge and felt on the porch roof attached to a rope lifeline. The attachment of the lifeline was tied around the truss and top plate of an interior wall to itself was not an approved anchorage system.
