29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about November 7th, 2018, at 1 Cobb Lane in Tarrytown; employees were working on a rooftop approximately 16 feet above ground level without any type of fall protection to include the use of a guardrail systems, safety net systems, or personal fall arrest systems.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  On or about November 7th, 2018, at 1 Cobb Lane in Tarrytown;  Employees working on a porch roof with multiple unprotected hole opening were exposed to a fall of 9 ft.

On or about November 7th, 2018, at 1 Cobb Lane in Tarrytown;   Employees exposed to a fall from a roof and/or holes were not provided with fall protection training.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.  On or about November 7th, 2018, at 1 Cobb Lane in Tarrytown  The portable telescoping aluminum extension ladder did not extend 3ft above the landing. The ladder extended approximately 3 inches above the roof edge.

29 CFR 1926.1053(b)(4): Ladder was used for purposes other than the purpose for which it was designed.  On or about November 7th, 2018, at 1 Cobb Lane in Tarrytown;  Employee stood on the 3rd rung from top of ladder. The Ladder Instruction Manual and warning labels on ladder stated not to stand or step on any rung within 3 feet of the top of the ladder.

On or about November 7th, 2018, at 1 Cobb Lane in Tarrytown;   Employees used a telescopic ladder improperly and ladder safety training was not  provided.
