29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. On or about November 2, 2018, the in-house double headed router was not guarded exposing employees to struck-by hazards.

29 CFR 1910.243(b)(2): Airhose. Hose and hose connections used for conducting compressed air to utilization equipment shall be designed for the pressure and service to which they are subjected. A screw/band hose clamp, which was not designed for use on an air hose, was attached on the airline hose for the air nozzle to blow off parts for the double headed router bit exposing employees to the whipping action of the hose if the clamp were to come off.
