On or about November 7, 2018, at a new building construction site located at 3433 S. Broadway in Edmond, OK, the employer allowed employees to enter a trench with spoil piles within 2 of the trench edge. This exposed employees to struck-by and crushed-by hazards.

On or about November 7, 2018, at a new building construction site located at 3433 S. Broadway in Edmond, OK, the employer did not ensure that protective devices were in place to protect employees working in a trench that was more than 5 feet in depth. The employees were exposed to crushed-by and struck-by hazards.
