29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: (a) Warehouse, Galaxy Line 8 - On or about October 30, 2018, the employer failed to provide effective training for repacker employees who clean conveyors to ensure the knowledge and skills required for the safe application, usage and removal of energy control devices were acquired. An employee sustained an injury to the right hand/wrist and arm while cleaning the infeed conveyor belt connected to the Harpak Ulma High Speed Wrapping Machine, Model Galaxy, Serial #2104536, while the conveyor was still operating.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) Warehouse, Galaxy Line 8 - On or about October 30, 2018, employees were exposed to machine hazards associated with moving parts and nip points when cleaning the infeed conveyor belt connected to the Harpak Ulma High Speed Wrapping Machine, Model Galaxy, Serial #2104536. The employer failed to implement energy control application steps as the conveyor was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)((i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.

29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in detail as required by the regulation: Anthony Marano Company, Chicago, IL - On or about November 6, 2019, the employer failed to record the following workplace injuries in the detail required by the regulation on the OSHA 300 Log for calendar year 2018. a. On or about October 1, 2018, a recordable injury occurred when an order picker employee fractured his left wrist after a fall to the concrete floor, resulting in 27 days of lost time. The information in Column F was not recorded in the detail required, as it did not described the object that directly injured the employee. b. On or about October 4, 2018, a recordable injury occurred when a repacker employee sustained a back strain while bending over to clean under a line, resulting in 40 days of lost time. The information in Column F was not recorded in the detail required, as it did not described the object that directly injured the employee. c. On or about October 8, 2018, a recordable injury occurred when a repacker employee received a contusion and laceration to the left index finger when the finger got stuck between rollers on the line when attempting to fix boxes that were stuck, resulting in 10 days of lost time. On or about October 25, 2018, a recordable injury occurred when a repacker employee strained the right shoulder and arm after a fall to the concrete floor, resulting in 5 days of lost time. On or about October 30, 2018, a recordable injury occurred when a repacker employee fractured the right wrist and injured the right arm when cleaning a conveyor line and making contact with a nip point on the conveyor, resulting in over 60 days of lost time.
