29 CFR 1910.29(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high.    1) On November 7, 2018, and at prior times, employees were exposed to fall hazards in excess of 10' when working or walking near the hoist area at the mezzanine storage area.  The employer had not ensured employees were protected from falls by a guardrail system as required by 1910.28(b)(2)(i)(A), in that a midrail chain was not installed.
