29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provided for an alternative fall protection measure:    a) Jobsite, Longfellow Drive, Munhall, PA 15120:  Observed on or about October 3, 2018, employees engaged in roofing activities on a roof with a slope of approximately 6 in 12 (vertical to horizontal) were not protected from falling by the use of guardrail systems, a safety net system, or personal fall arrest system.  The employees were exposed to a fall hazard of approximately 20 feet to the ground below.     J and R Titan Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1272595, citation number 1, item number 2 and was affirmed as a final order on December 8, 2017, with respect to a workplace located at 601 Longfellow Drive, Munhall, PA 15120.

29 CFR 1926.503(b)(1):  The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  a) Jobsite, Longfellow Drive, Munhall, PA 15120:  Observed on or about October 3, 2018, the employer did not maintain a written record of fall protection training for each employee working on the roof of the building.
