29 CFR 1918.81(i): Drafts were hoisted when the winch or crane operator could not clearly see the draft itself or the signals of the signalman who was observing the draft's movement: a) At the Port of Galveston, on the vessel Eleen Neptune, docked at Pier 34 on November 5, 2018, while granular fertilizer was being unloaded by the onboard crane, the employer failed to ensure that there was visual contact between the operator and the technicians working in the hull, exposing them to struck-by hazards.
