On or about November 6, 2018, during the installation of siding material at a job site located at 6633 Thistledown Cir NE, Canton Ohio 44721; the employer did not ensure employees working on a ladder jack scaffold were protected from a fall of approximately 23 feet by utilizing a fall protection system.
