Location: C/o Route 21 and Clay St., Newark, NJ.  a) Employees were engaged in staircase CMU construction operations were utilizing a tubular welded scaffold without any means of fall protection.  The scaffold did not have a guardrail system and the employees did not have a personal fall arrest system.  The employees were exposed to a potential fall hazard of up to approximately 40 ft. to the floor below; on or about 10/23/18.
