This condition exposed employees to caught-in hazards.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish to employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm. On or about November 8, 2018 and times prior, employees were exposed to caught-in hazards at the discharge chutes of the new MTC Meat Mixer - 0002 as a result of the safety interlocks on the machine guarding doors into the discharge area not working and causing the machine to inadvertently startup.
