29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye; a) 240 Main Street - An employee suffered an amputation of his right index finger to the 1st knuckle while stacking 58 lbs. of graphite blocks measuring 6 inches x 11 inches x 14 inches. The employee amputation occurred on October 24, 2018. The employer did not report the in-patient hospitalization to OSHA until OSHA called to inquire about the incident on October 29, 2018. No abatement certification or documentation required.
