OSH ACT of 1970 Section (5)(a)(1): Section 5(A)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or were likely to cause death or serious physical harm to employees in that employees were exposed to:     a) On or about November 8, 2018 and at times prior, a forklift operator was exposed to serious hazards while operating a TCM forklift at the production and warehouse area. The forklift was missing the seat belt that would assist in reducing the risk of entrapment of the operator's head and/or torso between the truck and ground in the event of a tip over.     Note: Among other methods, one feasible and acceptable method to correct this hazardous condition would be to follow the requirements for the use of an operator restraint system for powered industrial trucks at ASME B56.1-1993 - Safety Standard for Low Lift and High Lift Trucks.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a) On or about November 8, 2018 and at times prior, an employee was exposed to serious hazards at the production area from the point of operation of a CNC machine. The glass door of the CNC machine would not be suffice to prevent the employees from accessing the point of operation. The glass door could easily be opened while the machine was still in operation.

29 CFR 1910.37(b)(7): Each exit sign did not have the word "Exit" in plainly eligible letters not less than six inches (15.2 cm) high, with the principal strokes of the letters in the word "Exit" not less than three fourths of an inch (1.9 cm) wide:  a) On or about November 8, 2018 and at times prior, the word "Exit" on the exit signs were approximately 2 inches in height and the principal strokes of the letters were approximately one fourth of an inch. The exit signs would be difficult to see and exposing the employees to entrapment in the event of a fire emergency.
